Privacy Notice and Consent Form
GenesisCare patients can use the Prominet application (“app”) or via a webpage-based webapp to record and track their response to treatment on a device. .
When we set you up with this app/webapp we will collect your GC number, email address and, optionally, your phone number. We will also collect non-personal information automatically, such as your IP address. We will not process any other data. You will provide answers to questions about your response to treatment on a numerical rating with an option to supply text responses.
The lawful basis for processing your personal data in this app is consent: you accept this privacy notice and agree to take part in this survey. The survey results will be shared with GenesisCare in an anonymised format.
If you wish to have your data removed from the survey please contact your Centre Leader who will make the arrangements on your behalf.
